You can typically find this information on your driver's license, a birth certificate, or even a recent pay stub.
*Last Name:
Also known as surname; if your name is hyphenated, you can use both names. (Of course, you can just pick your favorite of the two. We promise we won't tell.) This might also be an alias.
*Desired User Name:
Be creative! You can choose virtually any username. Make sure to pick something you'll remember. You need this to log in. Need a suggestion? You can use your first name, last name, or both.
*Username is NOT case-sensitive*
*Password:
Choose wisely...you will need to remember your password to log in to your account. This should be simple for you to remember, but difficult for someone else to guess.
*PASSWORD IS CASE SENSITIVE*
*Confirm Password:
Re-enter your password exactly as you did in the previous field.
*PASSWORD IS CASE SENSITIVE*
*E-mail:
Enter the email address where you'd like to receive your confirmation email. Be sure it's an account you use regularly. If you lose your password, it can only be retrieved when we send an email to the account listed.
 
*Choose Security Question:
Choose a security question in the drop-down list, or select "Create My Own:" in the drop down list to make up your own question.
*Security Answer:
Enter an answer to your security question that you can easily remember. Please take note, as without this answer you cannot recover your password if ever lost.
